What the short forms collect
- Business name, your name, work email
- Phone on the call path (required there)
- Call / text consent checkbox when a phone is involved
- Optional notes you choose to share
- Which path you chose (callback or fit-test), plus optional guide answers when you use Find your fit
Short web forms are delivered to tiffany@3to99.com (subject [3to99 quote]) through our form provider, Formspree.
Census / spreadsheet path (Legal must-say)
- The blank census template is offered as a static file download from this site. Blanks do not contain employee personal data.
- You fill the spreadsheet offline. It asks for employer identifiers including EIN / Tax ID and employee census fields including tobacco Y/N. It does not ask for employee Social Security numbers (SSN).
- Filled census files are returned by email only to tiffany@3to99.com with subject exactly [3to99 census].
- This website does not provide an upload button and does not store filled census files in public webroot or static hosting.
- Filled census files come only by email to tiffany@3to99.com (subject [3to99 census]). They are not uploaded or stored on this website.
- We use the census to prepare a quote and follow up with you. We do not sell the information.
How we use it
We use what you send only to follow up on this call or quote inquiry — to answer you, prepare options, and take next steps you ask for. We do not sell it.
Calls and texts
When you check the consent box on the contact form, you agree we may contact you by phone or text about this request. We use that only to follow up on your inquiry — not for unrelated marketing. You can ask us to stop anytime.
Retention
We keep short-form and census inquiry information only as long as needed to prepare your quote and finish follow-up on this request, then delete or minimize what we no longer need. Filled census files are not stored on this website.
